About Elmore’s Buckinghamshire Support
With our Floating Support and High Intensity Need Support service, Elmore is working with people living in Buckinghamshire who have mental health needs but aren't getting mental health support from other NHS teams. Clients will likely be frequently accessing their GP and other primary care services. Staff meet with clients regularly to identify with them the areas they want support with and then look at how best this can be provided.
Commissioned through and in partnership with:

Adults (18+) living in Bucks and registered with a Bucks GP.
Clients will have ongoing mental health issues and be in frequent contact with primary care services, but will not be receiving ongoing support from other NHS mental health teams.

Workers will meet with you weekly initially for up to 12-weeks; for some individuals longer-term support of up to 2-years can be offered, where visits would be every fortnight instead.
Staff will meet you where you feel comfortable to meet, such as in your home or a local cafe, and work with you to address what you want support with.
This could be around mental health but doesn't have to be, and could focus on:
housing
finances/benefits
physical health
substance use
education/training
employment
advocacy

Currently the team is only taking referrals from specified health care teams in Buckinghamshire, including Mental Health Practitioners (MHPs) based in some GP surgeries.
